If it is air still in the system, where would the best place be to bleed it out? To bleed originally, I primed the diesel up with the filter pump, the cranked and opened the injector pipes at the injectors, when bubbles stopped I closed them and it started. Then with it running repeated bleeding the injector pipes but it did not get any better. Is there another place I need to bleed?

Yes, nothing has been changed and seems to function correctly as far as I can see

Looks like the standard Bosch one like mine. Throttle arm is different for the van and ignore the red lines - the photo was used for something else.

I have run out of fuel before and cranked it back into life without priming so if all your connections are good it should be working , unless its timing as obg mentiioned

It looks similar except the return banjo seem to be sitting up higher and a black pipe seems to be connected below it coming in from the right

It was from a 98 Caddy so was reasonably modern (although with split oil and water flanges it looked a mess was in very good nick)

SORTED!!!!, there seemed to be air getting in somewhere, as I bled it an it ran better for a minute then back to normal. The ford filter with primer was faulty and letting in air via a minute crack, I swapped it for the cheapo version filter and used my balloon primer, started first time and runs sweet! Happy Days. Thanks for all the advice and help guys

Glad it's sorted. Air in the system caused me starting problems for weeks until I found the problem. Mine was a crack in the filter where you connect the return pipe.

For future reference, Weldore was right about the issue I had. Same misfiring and horrible smoke just because I had put the banjos (not the pipes) on the wrong way (the return has a smaller hole and so I couldn't get enough fuel into the pump).
